Annual Summary of Work-related Injuries and Illnesses—OSHA 300A

An annual summary of occupational injuries and illnesses for each establishment must be posted. For the calendar, record work-related injuries and illnesses on the OSHA 300 Log and use this data to complete the OSHA 300A Summary. Enter the year covered, the company name, establishment name and address, annual average number of employees covered by the OSHA 300 Log, and the total hours worked by all employees covered by the OSHA Log. A company executive must certify that the information is correct and complete. Post the 300A from February 1 through April 30, beginning in 2003. If no injuries or illnesses occurred during the year, zeros must be entered on the totals line and the form must be posted for the same time period.
Accident Report Contractual Obligation
In addition to the requirement from OSHA that accident reports must be posted it is also a contractual requirement for contractors signatory with IBEW 701 to submit accident reports to the Safety Committee. The OSHA 300 A form may be used to fulfill your obligation to send accident reports.
